What is a SAR 7 form?
The Eligibility Status Report, also known as the SAR 7, is a report form that some CalWORKs (cash aid) and CalFresh households must complete, sign and send to the County, once a year. Benefits may be discontinued if verification of the information reported on the SAR 7 is not provided.
How do you fill out a SAR 7?
On the SAR 7, fill out the section to stop benefits only if you want to stop any of your benefits. Check the benefits you want stopped, and sign and date the SAR 7. If you only want to stop some of your benefits and keep others, you must fill out the rest of the SAR 7.
