Why does Ulquiorra have a second release?
Because he was actively disloyal to Aizen. It’s possible, though, that this wasn’t a tiny act of rebellion, but rather a big one – that Ulquiorra had some sort of secret plan to rebel against Aizen someday, and he was saving his second release for that time.
Why is Ichigo sword always in Shikai?

Shukai (主解, Mastered Release) is the third and final upgraded form of the Zanpakutō. It is the form of the Zanpakutō that displays both the Shinigami and the Zanpakutō’s true form and power. Like with Bankai, the Zanpakutō recieves a new name based on its upgraded abilities.
