The reason it is taking you so many hits to KO one thug (20 is probably an exaggeration) is because during the first 3 hits of a combo, Batman does almost no damage. Once your combo reaches x3, all subsequent hits, as long as you maintain your combo, will knock out regular thugs in only 2 hits.

While the game is rated “Teen” instead of “Mature,” there is a lot of combat action here. This includes up-close-and-personal melee attacks — kicks to the head, punches, acrobatic moves from above — as well as ranged attacks with weapons and gadgets. Some of these attacks are in slow motion to create a dramatic effect.

Well you definitely can, I bought arkham city on release with no idea that arkham asylum even existed. I suppose the situation is a bit different now with 3 games before AK, I’d highly recommend playing arkham city if you choose to play only one of them. It’s my favorite of the 3 games and AK is the direct sequel to it so it’ll make more sense.
