What is a Dapt score?

The DAPT score is a clinical prediction score assessing ischemic and bleeding risks at 12 to 30 months following percutaneous coronary intervention (PCI) to inform the duration of dual antiplatelet therapy (DAPT).

What is a cardiac MI?

A heart attack (myocardial infarction) happens when one or more areas of the heart muscle don’t get enough oxygen. This happens when blood flow to the heart muscle is blocked.

What are the causes of MI?

Myocardial infarction (MI) usually results from an imbalance in oxygen supply and demand, which is most often caused by plaque rupture with thrombus formation in an epicardial coronary artery, resulting in an acute reduction of blood supply to a portion of the myocardium.

How do I read my Dapt scores?

DAPT longer than 12 months is recommended for those with a DAPT score ≥ 2. A score of ≥2 is associated with a favorable benefit/risk ratio for prolonged DAPT while a score of <2 is associated with an unfavorable benefit/risk ratio.

Why is Dapt important?

A meta‐analysis by Udell et al showed that extended DAPT beyond 1 year among stabilized high‐risk patients with previous MI decreased the risk of major adverse cardiovascular events but at the cost of an increased risk of major bleeding.

How is emergency MI treated?

The pain of myocardial infarction is usually severe and requires potent opiate analgesia. Intravenous diamorphine 2.5–5 mg (repeated as necessary) is the drug of choice and is not only a powerful analgesic but also has a useful anxiolytic effect.

How many types of MI are there?

The three types of heart attacks are: ST segment elevation myocardial infarction (STEMI) non-ST segment elevation myocardial infarction (NSTEMI) coronary spasm, or unstable angina.

Who needs Dapt?

The American Heart Association secondary prevention guideline (2014) indicates that DAPT for 90 days in patients with severe intracranial stenosis is reasonable (Class IIb; Level of Evidence B), while patients with ischemic stroke/TIA due to atrial fibrillation who are unable to take oral anticoagulation can be treated …

When do you need Dapt?

Coronary artery stenting is done with either bare-metal stents or drug-eluting stents (DESs). The 2014 American Heart Association/American College of Cardiology (AHA/ACC) guidelines recommended DAPT in all patients with bare-metal coronary stents for at least 30 days after implantation.

When do you use Dapt?

In summary, DAPT should be started as soon as possible within 24 hours of minor ischaemic stroke or high risk transient ischaemic attack and should be continued up to 21 days. The 21st day is the possible trade-off point to balance treatment effect and bleeding risk.

How can you tell the difference between Type 1 and Type 2 MI?

For patients with type 1 MI, the focus is on aggressive antithrombotic therapy and consideration of urgent coronary angiography and revascularization. For patients with type 2 MI, the focus is on treating the extracardiac stressor precipitating the myocardial oxygen supply and demand imbalance.
